Helping you overcome funding hurdles

  1. Now that you have heard about augmentative communication, it is time to find out which device is best for you. In order to do this, you will need to see a licensed speech-language pathologist and get in touch with your local sales representative. Your sales representative can provide valuable help to you during this stage of the process.
  2. Once you have had an in-service or demonstration, as well as a speech therapy/augmentative communication evaluation session, your speech-language pathologist will need to write a report.
  3. You or your therapist will need to forward a copy of the speech evaluation to your physician. The physician will need to sign a prescription and complete any state Medicaid required forms. It is also helpful to ask your physician to write a letter of medical necessity.
  4. Complete the Client Information Form and sign all applicable release forms.
  5. Mail all documentation to DynaVox Technologies.
  6. If your therapist is recommending a trial period, or if your Medicaid program requires a trial period, you will need to complete and sign the rental contract.
  7. DynaVox Technologies will contact the funding source(s) for authorization.
  8. Once we receive a valid approval / authorization from all funding sources, we will ship the equipment. If you have a cost share due, you will need to pay this amount before the device ships. Your funding coordinator will contact you with the details.
  9. If your approval is for a trial period, we will ship a rental device to you for up to 4 weeks. At the end of the 4 week rental period you will then need to return the rental device to DynaVox Technologies.
  10. Your therapist will need to write a rental update report detailing the results of the trial period. If needed, you can request that the rental pages that were programmed onto the device be saved and re-loaded onto your new device, but be sure to inform your funding coordinator upon completion of the rental if you have requested these saved pages.
  11. Once we receive the rental update report, we will seek approval from the funding source(s) for purchase.
  12. When we receive a valid approval, we will ship your device.
